ERA-Bar Overdenture


This patient, a male in his sixth decade, presented with a hopeless situation involving the mandibular arch. The remaining teeth were severely periodontally involved and extraction was slated as the only reasonable course of action. The patient felt that he would not be happy with a complete lower denture so a course of dental implants was decided upon.

Four implants (Nobelpharma Conical Self-Tapping Implants) were placed in the mandible about three weeks after the extractions and the insertion of an immediate lower denture. The time delay was designed to allow for soft tissue healing and adjustment to the lower prosthesis. The lower denture was inserted with a soft tissue conditioner, Lynol, to help the patient adjust to this situation.

At three months post surgical, the implants were uncovered. Impressions were made of the implants using single piece implant impression copings (Impla-Med) and a bar was constructed using plastic UCLA abutments (Impla-Med) screwed directly to the implants. The bar was designed to house two distal ERA Partial Denture Attachments and one central ERA overdenture attachment (APM-Sterngold). A new lower overdenture was constructed and the male ERA attachments were placed in a conventional manner. The patient did well but requested additional retention so the orange males were substituted for the white males with a very satisfactory result.
